• 英語（平日8：30～17：00）　その他言語　翻訳アプリ使用をします。
• 院内での通訳の対応が出来ない場合は、通訳（若しくは医療通訳）の手配を依頼することがあります。医療通訳の費用が発生する場合は、患者さんの負担となります。

• English (weekdays, 8:30 a.m.-5:00 p.m.)  For other languages, a translation app will be used. 
• If an in-hospital interpreter is not available, we may request that an interpreter (or medical interpreter) be dispatched. Payment of any fees for a dispatched interpreter is the responsibility of the patient.

受診について / Important Information about Medical Consultations at Our Hospital

当院では診療申込時に全ての患者さんに対し、身分証明証（健康保険証、写真付きのパスポート・在留カードなど）の公的書類にて本人確認を行っております。ご来院の際はご持参ください。

Our hospital verifies the identity of all patients. Please present official identification (health insurance card, passport, Residence Card, etc.) when requesting a consultation. Please bring the necessary identification with you to the hospital.

日本の公的保険証をお持ちの患者さん / If you have Japanese public health insurance

• 保険証の提示をお願いします。
• 他の医療機関からの紹介状（診療情報提供書）が必要です。紹介状（診療情報提供書）がない場合は、選定療養費が別途かかります。

• Please present your health insurance card/certificate.
• A referral letter (patient referral document) from another medical institution is required. If you do not have a referral letter (patient referral document), you will be charged an additional fee as a patient without a referral.

日本の公的保険証をお持ちでない患者さん / Patients without Japanese public health insurance

• 来院目的の確認、支払いが可能かの確認をします。診療は全て自費診療となります。
• 受診は帰国を前提とした治療計画になります。
• 必要に応じ、英文診断書や診療情報提供書、搭乗許可証は有料で作成いたします。
• 医療滞在ビザ（日本において治療等を受けることを目的として訪日する外国人患者（人間ドックの受診者を含む）の受け入れはしておりません。

• We will confirm the purpose of your visit to the hospital and your ability to pay. You will be charged the full cost of your treatment.
• The medical treatment plan will be based on the assumption that you are returning to another country.
• There is a fee to issue a medical certificate or patient referral document in English or to issue a fit-to-fly certificate.
• Our hospital does not accept patients who are in Japan on a medical stay visa, including for comprehensive medical examinations (ningen dock).

支払い方法について / Payment

日本円でのお支払い、クレジットカードでのお支払いとなります。
クレジットカードでのお支払いの場合は、利用可能なクレジットカードか確認をいたします。
高額な費用が予想される場合には、クレジットカードの支払い上限を確認させていただきます。

Payment must be made in Japanese yen or with a credit card.
If you pay by credit card, we will check to make sure the card is accepted.
If treatment costs are expected to be high, we will check the limit on the card.

海外旅行保険に加入されている方は、一旦ご自身でのお支払いとなり、加入されている保険会社には、患者さん本人から連絡、対応をしていただきます。

If you have overseas travel insurance, you must first pay the full cost of treatment. You then need to contact the insurance company yourself and file a claim.
